Individual Counseling
Payments are due at time of service unless otherwise arranged with me. I require 24hrs notice for a cancelation. Late cancelations or no call no shows will result in full fee billed to client, which is not covered by insurance. Fees are subject to change.
Currently I am equipped to take cash, check, cards, and several insurances noted below. Do not let cost deter you from contacting me. I do offer a sliding scale for a limited number of clients and we can discuss treatment and/or referral as well. The bottom line is, if you have been brave enough to seek help I want you to get it. Even if we don’t mesh well, rates are too high, or I don’t have the right services for you, I can provide referral to outside agencies that may better suit your needs.
If you are paying out of pocket costs, I am required by law to provide you with a "good faith estimate". We will discuss and agree upon a cost and you will receive a signed copy estimating your cost per session.
Insurance
I am currently in network with Premera, Lifewise, Asuris, Molina, and Community Health Plan. I'm also in with a variety of others through contracting with Mindful Therapy Group and Headway (Look them up). Please check in with you insurance prior to our visit to make sure I am listed as a covered provider (you can call them or use the "find doctor/provider" search function). Sometimes, when not a lot of other providers are in network with your insurance, you can call the company and inquire about out of network coverage. Additionally, I can provide a "superbill" listing details about services we utilized, and you can try to turn it in to your insurance on your own.
Sliding Scale
While I may need proof of income – pay stub or W2 and signed attestation that the information is correct – you are able to roughly calculate what your cost may be based on poverty levels and my fees. NOTE I have limited space for sliding scale fee and the ultimate decision must be made in conversation with me. Federal Poverty guidelines can be viewed here:
